What is actually in the bag
A trial used a characterised extract. The bag you buy is a different thing, and sometimes a different plant.
safety
Every clinical trial on this site used a preparation that was specified,
standardised to a marker compound, and analysed. None of that is true of an unlabelled bag
of dried leaf, and the gap is where most herbal harm happens in practice.
The documented failure modes are consistent:
- Substitution. A product labelled as one plant containing another — sometimes a
- Contamination. Heavy metals, pesticides, and in imported material, undeclared
- Dose drift. A dried plant's active content varies with species, season, plant part,
What a household can actually do:
- Buy whole or coarsely cut material where you can, and look at it. Powder hides
- Prefer material you grew or gathered, and know the identification.
- Treat any imported "herbal" product marketed for a specific disease with particular
- Check for a batch analysis or a pharmacopoeial reference if buying a concentrated
What the studies found
Meta-Analysis2024Complementary therapies in medicine
Curcumin effects on glycaemic indices, lipid profile, blood pressure, inflammatory markers and anthropometric measurements of non-alcoholic fatty liver disease patients: A systematic review and meta-analysis of randomized clinical trials.
Curcumin supplementation in doses of 50-3000 mg/day over 8-12 weeks was associated with significant reductions in levels of FBG, HOMA-IR, TG, TC, LDL, weight and BMI in patients with NAFLD.

Toxic Metal in Plant-Based Therapeutics: Contamination Pathways, Oxidative Stress Induction, and Onset of Disease.
This comprehensive analysis underscores the urgent need for rigorous quality control measures throughout the herbal drug production process to mitigate metal-induced oxidative stress and its deleterious health effects.

Control compounds for preclinical drug-induced liver injury assessment: Consensus-driven systematic review by the ProEuroDILI network.
Extensive analysis highlighted the lack of agreement on control compounds for in vitro DILI assessment.

Therapeutic Effect of Natural Products and Dietary Supplements on Aflatoxin-Induced Nephropathy.
However, future research is warranted to translate the outcomes of this study into clinical use in kidney patients.

Profile of herbal and dietary supplements induced liver injury in Latin America: A systematic review of published reports.
This study underscores the challenge in causality assessment when multi-ingredients HDS are taken and the need for consistent publication practice when reporting hepatotoxicity cases due to HDS, to foster HDS liver safety particularly in LA.

Cautions and interactions
- Liver injury from a supplement is a diagnosis made by exclusion, and it is a
real risk. New jaundice, dark urine, pale stools or unexplained fatigue after
starting any herbal product means stop and get liver bloods checked.
- Do not buy concentrated extracts from a marketplace with no batch data. The
product with the strongest claim is frequently the one with the least analysis.
When this is not a self-care problem — get help
- Yellowing of the eyes or skin, dark urine, right-upper abdominal pain, or
unexplained bleeding and bruising after starting any herbal product — stop
it and seek medical assessment the same day.
